Making a claim for continuing items

Once your claim has been approved we require you to complete a Continuing Claim Form each month you are disabled or unemployed to continue obtaining benefits for each of the following:

  • a Disability claim;
  • an Involuntary Unemployment claim.

 

Continuing Claim Form - Disability

To submit a Continuing Claim Form for Disability you need to:

  • have completed the section headed Section A: Statement of claimant (you)
  • have your doctor or medical specialist complete the section headed Section B: Medical Certificate; and
  • sign and date the declaration.

Continuing Claim Form - Involuntary Unemployment

To submit a Continuing Claim for Involuntary Unemployment you must:

  • have completed the section headed Section A: Statement of claimant (you) on the claim form;
  • have a WINZ (Work & Income NZ) representative complete the section headed Section B: Statement of WINZ and stamp the appropriate box on the claim form;
  • provide a copy of your monthly benefits statement and
  • sign and date the declaration.
